Parental/Guardian Consent Form
Below is a link to the Parental/Guardian Consent Form. This is the letter that we ask you to sign to give your permission for your child to attend a trip either in the vicinity of the school or further afield. It ensures that you know where your child is and that the school is covered for insurance purposes. You can choose to complete the form that your child brings home; complete the form below and print out and send in or complete the form below and email back to office@stannes.cheshire.sch.uk
